Please take the test to help us determine if you should follow-up with a sleep specialist. When you submit the “test” by e-mail or mail, it will be reviewed by our specialist. We will then contact you by phone or e-mail to give you our recommendations.

The pre assessment tool the “test” is free of charge. Please note that if a follow-up is recommended, it is your choice to either make an appointment to see your personal physician or make an appointment with us. Any doctors visits or testing required as a result of the “test” will be billed accordingly.
